To register as a patient with Dr Showkat Surgery, please complete a registration form and email it to us at ggc.gp43542clinical@nhs.scot. You can also drop off the form at our reception.
Policy and Procedures to Registering as a Patient at Our Surgery
To register with our practice, patients must complete a registration form and bring it personally to the surgery, signed and accompanied by two forms of identification, such as a passport, driving licence, or ID card. You must be entitled to live in the UK, if there is any doubt, the practice may request further information.
It is essential that the form is filled out accurately and completely, including:
- Your date of entry to the UK
- Your previous GP and contact details
- Your previous address before moving to the UK
- Your place of birth
Failure to provide this information will delay your registration and may result in the form being declined. Providing correct contact details is important; if we cannot contact you, your registration may be delayed or declined.
All new patients are required to attend a health check with the practice nurse before seeing the GP. This process usually takes 3-4 weeks from the date the registration form is submitted.
On the day of your health check, please bring details of your previous medications. Failure to provide this information will delay the issuing of medication, as we cannot prescribe unless the medication is confirmed on your previous prescription records. We strongly advise requesting a one-month supply of medication from your previous GP before leaving their surgery, as we will not be able to provide this until we have received all relevant records.
